To the Pleasure Saddle Horse Registry: I hereby submit the above horse for entry in the Trail Award Program and certify that the information provided is true and correct to my knowledge. I also certify that I am the true and legal owner of the horse. I am 18 years or older. (If under 18, please ask parent or guardian to fill out the form.)

PLEASE NOTE: Application for entry must be made BEFORE the first hours are logged. All hourly logs for the year must be received no later than January 31st of the next year. In other words, for 2008 awards to count all logs must be received by January 31 of 2009. NO EXCEPTIONS! Get your Trail Log Sheets here.

APPLICATIONS: To register for this program, fill out the online form above or send the PSHR Trail Award Entry Form application in with the $25. (twenty-five dollars) per horse application fee made out to the Pleasure Saddle Horse Registry. Pay online with Paypal or US Mail it to: Pleasure Saddle Horse Registry - P.O.
